Consulting Engagement

Final quote provided after the intake consultation. No obligation until scope is confirmed in writing.
Payment terms: 50% on booking, 50% on delivery of the final planning summary. Payment accepted via Stripe (credit card, ACH transfer, or invoice).

Assumptions
This proposal assumes:
- The client has an active dialysis prescription and an established treatment schedule.
- Travel dates are flexible within a 2-week window to allow for scheduling around center availability.
- The client's nephrologist will provide required documentation in a timely manner.
- Destination center availability cannot be guaranteed — MAVTG verifies availability but does not control center capacity or staffing decisions.
